INSULATION - WARWICK TOWN HALL 12 ATHOL RD WARWICK MA 01378 The Town seeks price proposals from qualified building contractors. The deadline for submission is 7/15/2009 at 5 pm. Proposals may be e-mailed to coordinator@town.warwick.ma.us or mailed to Selectboard 12 Athol Rd Warwick MA 01378 or placed in the black mailbox in front of town hall. SCOPE OF WORK: The scope of work includes air sealing penetrations and installing cellulose insulation in a 2-story wood framed with clapboard siding town hall that has a 2562 sq. ft. footprint and a wall perimeter of 234 feet. The contractor will provide the materials, labor and equipment necessary for air sealing and installing dense pack dry cellulose insulation in the exterior walls and loose fill in the attic. * Air-seal all penetrations to maximize building performance. Existing fiberglass bats in attic must be moved to access penetrations. Door and window rough openings are excluded. * Air sealing for attic and basement penetrations and partition wall seams are to be done with two component spray foam. Large openings are to be done with foam board and with either one or two component spray foam as needed. * Install metal flashing or equivalent as needed to air seal chimney chases and elevator shaft in attic per building code. * Install dense pack national Fiber Cel-pak Cellulose or equivalent in exterior walls. Minimum density is 3.2 pounds per cubic foot to avoid settling. Installation to be from exterior after removal of clapboards where possible. Work to be coordinated with painting contractor. * Re-install fiberglass bats in attic if re-usable. * Install 10 settled inches of loose fill National fiber Cel-pak Cellulose or equivalent in attic over or under existing fiberglass bats. SITE VISIT: It is the responsibility of the bidder to make a site visit to the Warwick Town Hall, 12 Athol Rd Warwick MA 01378. You may contact J. PREVAILING WAGE REQUIREMENTS: This is a prevailing wage job. Only sole proprietors without employees may be exempt. PAYMENT BOND: The successful firm is required to submit payment bond as required by Massachusetts GL 149 Section 29 in the amount of 50% of the contract price. INSURANCE REQUIREMENTS: Liability insurance coverage will be considered as primary and not as excess insurance. The vendor will provide thirty - (30) days written notice to the Town by registered mail prior to any modification, cancellation, non-renewal or other change in coverage. The policies must be effective prior to the commencement of work and must remain in force until termination of work under this contract. In the event of interruption of coverage for any reason, all work under the contract will cease and will not resume until coverage has been restored. If at any time during the term of this contract or any extension thereof, any required policies of insurance should expire, or are canceled, it will be the responsibility of the Contractor to furnish to the Town a Certificate of Insurance indicating renewal or an acceptable replacement of the expiring policy prior to the expiration or cancellation date so that there will be no lapse in any coverage. The Town will be named as an additional insured. The following minimum coverage is generally required of vendors providing services: Workers Compensation Applicable State Statutory Employers' Liability $100,000.00 Contractor's Liability Insurance Form of insurance will be by a Comprehensive General liability and Comprehensive Automobile Liability. To the fullest extent permitted by law, the vendor agrees to defend, pay in behalf of, and hold harmless the Town of Warwick, its elected and appointed officials, employees and volunteers and others working in behalf of the Town, against any claims, demands, suits, loss, including all costs connected therewith, for any damages which may be asserted, claimed or recovered against or from the Vendor, by reason of personal injury, including bodily injury and death; and/or property damage, including loss of use thereof, which arises out of or is any way connected or associated with this contract. INDEMNIFICATION The successful firm shall hold Town of Warwick harmless from and indemnify it against all liability, including attorney's fee, which may arise from and accrue directly from the performance of the work or any obligation of the successful firm or failure of the successful firm to perform any work or obligation provided for in the agreement. PAYMENT Payment in the amount of 100% of the contract shall be paid upon final completion of all requirements specified within this document. METHOD OF AWARD The contract will be awarded to the lowest responsible and responsive responder. The Town of Warwick reserves the right to select the quote that best meets its required needs and quality requirements, to reject any or all bids, and to waive any formalities. OTHER CONDITIONS All work will be guaranteed for one year from the date of completion. Any changes require prior approval by the Town of Warwick.
